Please fill out the form completely. Failure to provide correct information will delay processing of the International Health Certificate for your pet and possibly delay your travel plans. If you do not have all of the required information at this time, complete as much as possible.

This form needs to be returned to our office no later than two weeks prior to your appointment. We reserve the right to cancel your appointment if not received on time. Please call us if you need help completing this form.
